Hello doctor,

I am a 73-year-old woman who has been experiencing recurrent bacterial cystitis since my youth. Initially, I had episodes of hemorrhagic cystitis, but over time the blood emissions resolved. Currently, I am experiencing pain in my right-sided back, along with chills and a slight fever, coinciding with bacterial cystitis episodes that require treatment with antibiotics. Before urination, I also experience a distressing pain and burning sensation, as well as frequent urination due to a sense of urinary urgency. Additionally, I have an electric sensation that originates from the right side of my back and extends to my toes. For a discopathy issue, I have undergone two MRI scans of my vertebral column. The radiologist noted abnormalities in the size and position of my kidneys during these scans. Kindly review the attached MRI images to determine if there are any medical issues with my kidneys and if further imaging tests, such as an MRI or CT scan of the kidneys, are necessary. Can you provide a possible diagnosis for my symptoms? If not, which additional tests (blood, urine, imaging) should I undergo? Would an MRI or CT imaging be more suitable?

Recurrent cystitis can have various underlying causes, including renal stones, decreased bladder contraction leading to urine stagnation and infection, diabetes, poor hygiene, and post-menopause factors. You have a history of urinary tract infections since your youth. It is possible that genetic factors such as a small bladder or vesicoureteral reflux contribute to recurrent urinary tract infections, which can subsequently affect the kidneys. To provide a more accurate assessment of your kidneys, I suggest you share the final report from the radiologist of your MRI (magnetic resonance imaging). I suggest you undergo an ultrasound of the kidneys or directly proceeding with a non-contrast computed tomography (NCCT) scan of the kidneys. These imaging tests will provide valuable information regarding the presence of kidney stones, kidney size, and potential kidney infections. I hope this has helped you. I do not have the MRI report for the kidneys because it was reviewed by one of my friends who is a physician, and they informed me about the findings. Can you see anything in the attached images at all? Kindly provide your insight.
