How to get rid of dark patches on the hands and toes?

Hello doctor,
I am a 21-year-old female. I develop dark discolored rough patches on my hands and toes. No matter what I do to remove them they come back worse and darker than before. It ruins my self-confidence as a woman who should have pretty hands and feet.

I have seen your query and concern and examined your picture (attachment hidden to protect the identity of the patient). With your descriptions and given picture, it seems you have callosity or lichen simplex chronicus. I would suggest you apply Xerina cream (glycolic acid; urea; cetylated fatty ester complex) or Moisturex cream (lactic acid, liquid paraffin, propylene glycol, and urea). Wait for 15 minutes then apply topical steroids with salicylic acid (Propysalic NF lotion. Occlusive dressing will be more helpful; after the application of Propysalic lotion, cover with a flat polythene cover, then a cotton bandage. Leave it for the whole night for two to three days, occlusion is not needed in the daytime) and should be applied twice daily for two to four weeks. Also, take one tablet of Levocetirizine once daily for a similar duration.
Otherwise, you can try an injection of Kenacort (Triamcinolone acetonide) 10 mg /ml 0.1 ml/cm2 intralesional at the three-week interval should be given by a dermatologist. All medicine should be used after consulting your general practitioner or nearby dermatologist.
