Why are my periods late by weeks despite emergency pills?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am currently about two weeks late for my period. My last menstrual cycle occurred in the third week of the previous month. I had unprotected intercourse once earlier in the cycle and took emergency contraception immediately (two pills taken 12 hours apart). I had intercourse again twice toward the end of the cycle and took emergency contraception again after the last encounter (two pills taken 12 hours apart).

My menstrual cycles have been irregular for the past few months, usually lasting between 35 and 42 days. This irregularity started after a miscarriage earlier last year. I recently used a home pregnancy test and noticed a faint second line. I would like to know if this could indicate pregnancy and what medical options are available if the pregnancy is unintended.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have read your query and can understand your concern.

Based on the dates you provided, pregnancy is possible despite taking emergency pills, because emergency contraception is less effective when ovulation has already occurred, and repeated use in the same cycle can also disrupt hormones and delay periods.

Emergency pills can cause cycle delay, spotting, or false reassurance, but a faint second line on a pregnancy test should be considered positive until proven otherwise, especially since you are now 13 days late.

The correct next step is not to take more emergency pills, as they will not work at this stage and may worsen the hormonal imbalance. You should repeat a urine pregnancy test using first morning urine, or preferably get a blood beta hCG test (beta human chorionic gonadotropin test) to confirm pregnancy. If pregnancy is confirmed and it is unwanted, you need to consult a qualified gynecologist as soon as possible to discuss safe and legal options based on gestational age, rather than trying to manage this on your own.

Your history of irregular cycles after miscarriage (spontaneous loss of a pregnancy before the 20th week of gestation) increases uncertainty about ovulation timing, which explains why emergency pills may have failed. Please seek in-person medical care promptly, as early evaluation gives you the safest options and protects your health.

I hope this answers your query.

Please let me know if I can assist you further.

Thank you.
