Hi doctor,

My period was eight days late, but when it came, it was like spotting and was done after two to three days.

What should I do? I have back pain, a headache, nipple pain, and bloating. Please help.

  1. Heavy vaginal bleeding, soaking more than one pad per hour, or passage of large clots.

  2. Severe or worsening lower abdominal and pelvic pain.

  3. Missed periods for more than two consecutive cycles.

  4. Positive pregnancy test or suspicion of pregnancy.

  5. Severe dizziness, fainting, or extreme fatigue.

  6. Persistent or severe breast pain, nipple discharge.

  7. Fever or signs of infection.

  8. Sudden, rapid weight loss or gain.

  9. Recurrent cycle irregularity lasting more than three months.

Causes can be:

  1. Stress-related cycle disturbance.

  2. Early hormonal imbalance.

  3. Early pregnancy.

  4. Thyroid or lifestyle-related cycle variation.

You can go for the following investigations:

  1. Urine pregnancy test.

  2. CBC (complete blood count).

  3. Free serum T4 (thyroxine), serum TSH (thyroid-stimulating hormone), and serum prolactin.

  4. USG (ultrasonography) pelvis for uterus and adnexa.

Take the following preventive measures:

  1. This pattern is common at 18 years and is usually self-correcting.

  2. Maintain regular sleep, a balanced diet, and stress management.

  3. Avoid a crash diet or excessive exercise. Weight in the optimum range is imperative for healthy menses.

  4. Track your cycles using a menstrual calendar.
