What causes delayed periods with hot flashes and tiredness?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I have not had a period for the last two months without pregnancy. I also have hot flashes, tiredness, mood swings, and joint pain.

Please guide.

Hi,

Welcome to icliniq.com.

I read your query and understand your concerns.

It may be due to a premenopausal state. If you have not done a pregnancy test, please do one first. If the result is negative, you will need further tests.

You will need to undergo the following blood tests:

  1. CBC (complete blood count).

  2. PP sugar (postprandial sugar).

  3. TSH (thyroid-stimulating hormone).

  4. Prolactin.

  5. Testosterone.

  6. Beta hCG (beta human chorionic gonadotropin).

  7. FSH (follicle-stimulating hormone).

Treatment will be decided based on the results. Additionally, you will need to have a transvaginal ultrasound (TVS) of the pelvis. Your irregular periods may also be related to certain medications you are taking.

  1. Are you on any medications or supplements for any reason?

  2. Are you going to the gym?

Symptoms like hot flashes, fatigue, and mood swings suggest that you may be nearing menopause. Many women experience menopause around the age of 40.

It is important to note that psychological stress or other forms of stress can also lead to amenorrhea (the absence of menstruation). Have you ever taken an emergency contraceptive pill? Some women may not get their periods after taking it.

  1. Regarding your joint pain, could you clarify where it is occurring?

  2. Is it in the small joints, like your fingers, wrists, or toes, or larger joints like your knees or shoulders?

  3. Do you experience stiffness in your joints in the morning?

In any case, you should proceed with the tests as advised.

I hope this answers your query. Feel free to reach out anytime.

Thank you.
