What causes missed periods despite a negative pregnancy test?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I hope everything is good. Doctor, I have irregular periods again; for three months, I have not had a period. We did not have any intercourse. But when I check the pregnancy test, it is negative. Can you suggest any medicine, please?

Kindly help.

Hello,

Welcome to icliniq.com.

I understand your concern.

It sounds like you are dealing with irregular periods again, and you have missed your period for three months. Since you have taken a negative pregnancy test, it is good to rule out pregnancy. The possible causes for your missed periods and what you can do next are discussed below.

Irregular periods can result from hormonal imbalances caused by conditions such as Polycystic Ovary Syndrome (PCOS), thyroid issues, stress, or lifestyle changes like alterations in diet, exercise, or weight. Low body weight or excessive exercise can also affect hormone production, leading to missed periods. Emotional factors such as high levels of stress or significant life changes can further disrupt the hormonal balance responsible for your menstrual cycle. If you are in your late 30s or early 40s, irregular periods may indicate perimenopause, a phase that causes hormonal fluctuations.

If your periods continue to be irregular for several months, it is important to consult a gynecologist. They may recommend tests such as hormone level assessments (including thyroid, prolactin, estrogen, and progesterone) or a pelvic ultrasound to check for Polycystic Ovary Syndrome (PCOS) or cysts. Progestin therapy or a progesterone challenge is often prescribed to help induce a period, typically taken for 10 to 14 days, with menstruation expected to start within three to seven days after completing the medication. Birth control pills are commonly used to regulate cycles if hormonal imbalance is the cause. Maintaining a healthy diet, managing stress levels, and following a moderate exercise routine can also help improve cycle regularity. Tracking your cycle or using an ovulation predictor kit may help identify patterns in your menstrual irregularity. If missed periods persist and are accompanied by severe symptoms such as weight gain, hair loss, or acne, it could signal a hormonal condition that requires medical attention.

I hope this helps you.

Thank you.
