Why is my wound not healing after stitch removal?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am suffering from wound dehiscence after stitch removal. My wound has opened. Now I take Augmentin 625 BD and PCM 500 BD and apply Metrogyl M ointment twice, but there is no relief.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have gone through your query and understand your concern.

You have wound dehiscence (wound opening) after stitch removal. It can be superficial or deep wound dehiscence.

Superficial wound dehiscence involves only the skin and subcutaneous fat layers, which are separated with minimal wound gaping and usually need only dressing with normal saline if not infected. If it is infected, then culture and sensitivity testing are needed, and healing is allowed by secondary intention.

Deep wound dehiscence involves fascial layers and muscles. It can be closed if it is a clean wound; if not, proper wound management is required.

There are factors that can cause wound dehiscence from the patient's side, the surgeon's side, and the suture materials used. Conditions like diabetes (high blood sugar levels), tight wound closure, malnutrition (an imbalance between the nutrients your body needs to function and the nutrients it gets), anemia (when you have low levels of healthy red blood cells to carry oxygen throughout your body), and infection can lead to wound dehiscence. In case it is a clean wound, you do not need to use antibiotics.

I would like to ask you, please do not mind, in order to guide you with the best treatment approach:

  1. What type of surgery?

  2. After how many days were the stitches removed?

  3. Can you share a picture of the wound?

  4. Any medical illnesses or medications that you are currently using other than those you mentioned?

Please kindly answer these queries for proper guidance and treatment.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.
