Hello doctor,
My 25-year-old wife has had Type 1 diabetes since she was 18. She uses an insulin pump and wears a continuous glucose monitor (CGM). Her latest Hemoglobin A1c is 6.9 percent. However, she also has PCOS (polycystic ovary syndrome) and takes Metformin for it, in addition to her insulin. We have been trying to conceive for a year without success, and she is worried that her diabetes or PCOS could be affecting our chances. Should we see an endocrinologist or a fertility specialist? She also sometimes experiences severe lows at night, and her CGM alarms wake her up. Is there a way to adjust her pump settings to prevent these low blood sugar episodes? Are there any dietary or lifestyle changes she should consider while managing both conditions?
