What IV infusion can help prevent diarrhea in my 1-year-old?
Patient's Query
Hello doctor,
My one-year-old has had watery stools for about two weeks. She was vomiting earlier, but that has now stopped. She is no longer breastfeeding. I would like to ask, what injection or infusion can be used to prevent diarrhea? She is currently receiving a normal saline infusion.
Kindly suggest.
Hello,
Welcome to icliniq.com.
Thank you for providing detailed information.
I understand that your one-year-old child has been experiencing watery diarrhea for approximately two weeks, with vomiting that has since resolved. She is no longer breastfeeding and is currently receiving a normal saline infusion. At this stage, it is important to transition from normal saline to a more suitable maintenance fluid.
If the child has adequate urine output, 5% dextrose with 0.45% sodium chloride (NaCl) and 20 milliequivalents per liter of potassium chloride (KCl) is recommended. If there are signs of metabolic acidosis, Ringer’s lactate may be more appropriate.
Zinc supplementation should be provided, as it can help reduce the duration and severity of diarrhea. The use of probiotics, such as Saccharomyces boulardii or Lactobacillus species, can also be considered to support gut recovery.
Antibiotics are generally not indicated unless there are signs of an invasive bacterial infection, such as blood in the stool, persistent fever, or systemic symptoms. Oral rehydration with an oral rehydration solution (ORS) should be continued between intravenous fluid sessions if the child can tolerate it.
A bland, age-appropriate diet may be reintroduced, including foods such as rice, bananas, and mashed potatoes. It is advisable to avoid dairy products and sweetened beverages, as temporary lactose intolerance may occur.
If not already done, stool tests and serum electrolyte levels should be checked. The child’s weight and hydration status should also be monitored closely. Referral to a pediatrician or hospital admission should be considered if there are signs of dehydration, persistent high-output diarrhea, or poor feeding, or if the condition fails to improve within the next two to three days.
I hope this information is helpful.
