Hello doctor,

I am a 28-year-old male weighing 280 pounds and 5 feet 8 inches tall. I have been experiencing difficulty urinating lately. Sometimes in the morning, it takes a few seconds for the urine to come out, but I can feel the flow fine until it stops at the penis head. Occasionally, it burns, but not always. When I do urinate eventually, it sometimes sprays out randomly until I push it out a bit harder. This has been happening on and off for a few months. It feels like my urethra is swollen. I am not taking any medications. Please help.

Do you have a history of a urine infection in the last year or so? Any history of urethral discharge or kidney stones? Is there a recent history of unprotected intercourse? The symptoms suggest an infection or swelling of the prostate. This can be checked by a rectal exam, a uroflow test, sonography of the kidneys and prostate, urine culture, and a urine routine or microscopy.

Kindly get these tests done. Meanwhile, you can ask for a prescription for an alpha-blocker (such as Alfuzosin) and some anti-inflammatory medication. Once the infection is confirmed, a course of antibiotics will be needed for about one month.

Hello doctor,

Thank you for replying.

I have no known urinary tract infections, no urethral discharge, and no history of kidney stones. Yes, there has been recent unprotected sex with my wife, who is my only partner. It does not seem like it would be prostate-related, but you would know better. I can feel my urine traveling up the urethra and stopping at the tip of the penis. I will look into getting those tests done.

With prostate swelling, you do experience such symptoms. Often, most tests come out negative, but a good rectal exam will help diagnose it. Till then, I would suggest you take Alfuzosin 10 mg once daily from your doctor or try herbal medicines for the prostate (over the counter) that contain saw palmetto. Anti-inflammatory medications like Ibuprofen can also help for a few days. Once the reports are available, feel free to ask me or your doctor for a prescription.
