Hello doctor,

My wife had an ectopic pregnancy earlier this year. We had to remove her fallopian tube and remove the pregnancy. We just got married. I read somewhere that the Postpone-72 tablet is risky if you have ever experienced tubal pregnancy before. We need some advice. Should she take it?

I understand your concern.

There are few reported cases of ectopic pregnancy following the use of Postpone-72 (Levonorgestrel). However, the very low risk of having ectopic pregnancy following Postpone-72 (Levonorgestrel) occurs if the drug is taken during the ovulation. If the drug is not taken around this period, the minimal risk of ectopic pregnancy is eliminated. The most important thing for her, since she has had a previous ectopic pregnancy, is to do a transvaginal ultrasound immediately after she misses her period or suspects she is pregnant to determine the location of the pregnancy. This is important because she has close to 15 % risk of having a second ectopic pregnancy. Thus, there is a need for early localization of subsequent pregnancy so that if it is an ectopic, a tubal conservative option of surgery can be offered to her. Postpone-72 (Levonorgestrel) only has a minimal risk of causing ectopic pregnancy. So, if you are not ready for pregnancy, she can go ahead and take it.

If her last period was a few days back, it is most likely around her ovulation period. There is little risk of developing ectopic pregnancy if the Postpone-72 (Levonorgestrel) is taken. However, this risk is minimal compared to the risk of having an ectopic she already has a previous history of tubal ectopic pregnancy. The most important thing for her is an early ultrasound scan when she misses her period for localization of pregnancy. Taking the drug, the risk of developing ectopic is four in about 100 cases. You also need to know that Postpone-72 (Levonorgestrel) should be taken with 72 hours of unprotected vaginal penetrative and ejaculatory sexual intercourse.
