Hi doctor,

I am suffering from occasional swelling of epididymis on the right side. It usually swells once or twice in a month causing acute discomfort. The duration of swelling ranges from 30 minutes to 2 hours, after that, it fades away. Along with epididymis, the right scrotum also swells. This problem started a few years ago. It started one day after I masturbated for a long duration (probably 30 minutes). After that, I went for a six months course at a hospital. They did some tests initially but nothing was found. After taking a three month course of medicines, initially Alfusin, Inflapen, and Punox, then capsule Dutas, Zevit, and finally tablet Punox, and Rantal. After some tests, mild orchitis is found (some fluid was seen in both the scrotal sacs). But even after taking heavy doses of the said medicines, the problem did not resolve completely. After that, I shifted to my hometown. The problem persists occasionally. Six months ago, the acute problem started when it started swelling on alternate days. I got a check-up again and took some antibiotics, and did multiple tests and ultrasounds. But all the tests and ultrasounds came out good with no problem. The doctor said there is no case of epididymitis or orchitis or any other urinary-related disease. I was not taking any antibiotics or medicine, just used to take up one tablet for swelling (Lyser D) only when it swells too much. I am married for six months now and also planning to start a family. Please suggest some solutions and also tell is it going to affect my fertility? One important thing I want to mention here is that nowadays it is swelling mostly after the excitement and if I ejaculate the sperms then the swelling, as well as the pain, fades away. If I do not ejaculate, then the problem persists for four to five hours and then naturally fades away.

Your reports (attachments removed to protect patient's identity) are very clear that you do not have epididymitis or as you mentioned no urinary symptoms. I do not think you will have fertility problems and no benefit of discussion before one year of sexually regular active life without any contraception. If any specific queries are there then we will discuss them.
