Is CBD oil safe and effective for managing epilepsy?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello Doctor,

I am 33 and was diagnosed with epilepsy two years ago after experiencing breakthrough seizures. I have been on three different medications, but I still have seizures every few months. The most recent episode happened during my son’s birthday party and traumatized all the kids. My wife is getting frustrated because I can not drive reliably, and she has to handle all the transportation for our two children.

I work in construction, and my employer is losing patience with my medical leave absences. The seizures usually happen when I am tired or stressed, but it is hard to avoid those triggers with work and family responsibilities. I have gained 40 pounds from these medications, and my memory has gotten much worse. Sometimes I forget conversations I had just the day before.

My neurologist mentioned surgery, but that scares me even more than the seizures. What if something goes wrong and I can not work at all? We can not afford for me to be disabled. I have been researching CBD (Cannabidiol) oil and other alternative treatments, but I do not know which ones are legitimate.

My father-in-law keeps suggesting that I am not trying hard enough to control the seizures. Is there any way to make the medications work better, or are there newer treatment options available?

Kindly help.

Hello,

Welcome to icliniq.com.

I understand your concern.

If you have tried two or more appropriately chosen and tolerated anti-seizure medications without achieving seizure control, it is considered drug-resistant epilepsy. This does not mean your condition is untreatable; it means you need to be evaluated at a comprehensive epilepsy center, where a more specialized workup can help guide effective treatment.

Surgery might sound extreme, but for many people with focal epilepsy, it is the most effective path to long-term seizure freedom, offering a 50 to 70 percent chance of control. Thanks to modern evaluations like video EEG (electroencephalogram) monitoring, MRI (magnetic resonance imaging), PET (polyethylene terephthalate) scans, and functional mapping, doctors can precisely locate the seizure focus and assess the associated risks. If the seizure source is in a non-critical area of the brain, surgery might not only reduce seizures but also improve memory, cognition, and overall quality of life. Some patients may also qualify for less invasive treatments such as laser interstitial thermal therapy (LITT) or neuromodulation devices that modulate seizure activity without removing brain tissue. Rest assured, epilepsy centers take time to fully evaluate your case before recommending surgery, they ensure the benefits outweigh the risks.

The cognitive and physical side effects of anti-seizure medications, especially memory loss, fatigue, and weight gain, are real and impactful. A medication reassessment or a new combination approach may help, particularly when guided by specialists who focus on difficult-to-treat epilepsy.

CBD oil (specifically Epidiolex) is FDA (Food and Drug Administration) approved for certain seizure types, but its off-label use for focal epilepsy is still under study. While it is not a miracle cure, some adults do experience benefits when it is used as an add-on therapy. If you are interested, talk to your neurologist about trying it and avoid unregulated versions sold online. Diet-based therapies, such as the ketogenic or modified Atkins diet, have also shown positive results in adults with drug-resistant epilepsy but must be medically supervised to avoid complications. Lastly, lifestyle management remains essential: prioritizing good sleep, reducing stress, and avoiding alcohol can make a significant difference. Counseling or therapy may also help, especially since stress is a known seizure trigger.

I hope this helps.

Thank you.
