What causes loss of erection despite an active sex life?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello Doctor,

I have been sexually active for the past three years. I was in a relationship with a married woman who wanted my help to have a baby. At first, I was afraid to have sex with her because I was worried her husband might find out. But later, I was able to do well, and I could last around five to ten minutes at most. I did have difficulty getting an erection the second time.

Even now, when some of my female friends come to my room, I am not able to perform the way I want to. The main issue is that when I try to insert my penis, I lose my erection. It is hard at first, but I cannot stay hard, and I am not able to get erect again afterward.

I used to masturbate twice a day, but I stopped doing that about a month ago because I started meeting women for sex. Still, I am not able to perform the way I would like. The women I have been with are not making fun of me, but they do say that I seem to lose focus or excitement during sex and that I might improve with more experience in the future.

Kindly help.

Hello,

Welcome to icliniq.com.

I understand your concern.

Based on what you have shared, you may be experiencing erectile dysfunction (ED). Here are some common reasons for ED and what you can do to improve your condition:

Possible causes of erectile dysfunction:

  1. Stress from work, family, or irregular sleep (like night shifts). Try regular exercise and yoga to reduce stress.

  2. Depression or anxiety.

  3. Varicocele (enlarged veins in the scrotum). An ultrasound of the scrotum can help check for this.

  4. Fear of failure, one bad experience, can create worry, which makes the problem worse.

  5. Low self-confidence, often caused by frequent masturbation.

  6. Not being sexually attracted to the opposite sex.

  7. Cultural or religious guilt related to sex.

  8. Smoking and alcohol use.

  9. Chronic illnesses such as heart disease, thyroid problems, kidney disease, or long-term medications.

  10. Vitamin deficiency, taking supplements like Zincovit (multivitamin), and calcium carbonate may help.

  11. Negative feelings toward your partner.

  12. Hormonal issues, including low testosterone, thyroid problems, or high prolactin levels.

  13. Long-term use of certain medicines.

  14. Diabetes and high blood pressure.

Tests you may need:

  1. Doppler test of the penis.

  2. Ultrasound of the pelvis and scrotum.

  3. Blood pressure check.

  4. Blood tests: blood sugar (RBS), thyroid (Free T3, T4, TSH), testosterone, prolactin.

What you can do:

  1. Exercise regularly and eat a balanced diet.

  2. Basic exercises like Kegels, squats, yoga, running, or brisk walking for at least five minutes a day.

  3. Sex-position specific exercises: Missionary position: planks, push-ups, mountain climbers, glute bridges, standing position: bicep curls, pull-ups, jump squats, doggy style: walking lunges, jump squats, cowgirl position: leg raises, glute bridges.

You can include strawberries, apples, avocados, sweet potatoes, sesame seeds, almonds, garlic, and a small amount of dark chocolate in your diet.

You can try a tablet called Mildfil 5mg (mention generic name), twice a day, but only after consulting a doctor. If you can share more details about your lifestyle, stress levels, health, and habits, it will be easier to understand the exact cause and suggest better treatment.

I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.

Thank you.
