Hello doctor,

I am experiencing an external extrusion from my anal side. There is no pain or itchiness. I generally put it back in and readjust it. I would like to know the severity and treatment options for this particular issue.

I have reviewed your case history and attached pictures (attachments removed to protect the patient's identity). So, these are external hemorrhoids (piles) which are likely grade 3, if they come out of the anal verge and you have to manually reduce them. Medical treatment includes avoiding constipation (as you already do or no constipation), and continuing taking a high fiber diet. If it would cause bleeding or pain at the anal verge, or you feel it is relatively difficult to insert it back, this would need surgical treatment for hemorrhoids. Ideally, you need a local examination of the anal canal by a general surgeon or Gastroenterologist, a short clinical procedure known as proctoscopy, to establish this diagnosis.
