How can fever in toddlers with seizure history be safely managed?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am seeking advice regarding my 18-month-old son. He experienced a seizure at 8 months old. Since then, as per our doctor's prescription, we administer Clobazam (Cloba) along with fever syrup when he develops a fever.

Occasionally, his fever reaches 102°F. I have a couple of questions:

  1. Is it appropriate to give him Ibugesic plus every six hours if his fever returns?

  2. After administering Ibugesic plus, at what point (in terms of time and temperature) should we consider adding Mefenamic Acid (Meftal P) syrup to manage high fever?

Your guidance on the correct dosage and timing for these medications would be greatly appreciated to ensure his safety and comfort.

Please help.

Hi,

Welcome to icliniq.com.

I understand your concern.

To effectively manage a fever, consider the following plan:

  1. Paracetamol syrup (125 mg/5 mL): Administer 5 mL at the onset of fever. If necessary, repeat every four hours. Paracetamol is an antipyretic that helps reduce fever by inhibiting the production of prostaglandins, chemicals in the body involved in inflammation and fever.

  2. Ibugesic syrup (100 mg/5 mL): If the fever persists an hour after Paracetamol or recurs before four hours, administer 4 mL. This can be repeated every eight hours if required. Ibugesic contains Ibuprofen, a nonsteroidal anti-inflammatory drug (NSAID) that reduces fever, pain, and inflammation by inhibiting cyclooxygenase enzymes (proteins that are very important to the body help turn a fatty acid found in cell walls. These chemicals are very important for many bodily functions, such as inflammation, pain, heat, and blood clotting) involved in prostaglandin synthesis.

Follow-up recommendations:

  1. Hydration: Ensure adequate fluid intake, including water, milk, juices, and soups. Staying hydrated helps regulate body temperature and supports overall health.

  2. Identify underlying causes: Please note that, without additional information, it is challenging to identify the underlying cause of the fever. If the fever persists beyond a few days or is accompanied by other symptoms, consult a healthcare professional for a thorough evaluation.

I hope this helps.

Kindly revert so I can assist you further.

Thank you.
