Hello doctor,

I had an unprotected sexual encounter five days ago with a partner who claimed not to have had any previous partners. I felt a little discomfort at the tip of my penis two days later and, upon checking, noticed a small ulcer at the tip. Yesterday, it was the same size but white in color, similar to a mouth ulcer. Should I be worried about an STI?

It appears to be a sexually transmitted infection. Apply Fucidin cream (Fusidic acid) twice daily for seven days. I suggest you take 500 mg of Ciprofloxacin (a fluoroquinolone antibiotic) once a day after eating food for five days.

Here are some suggestions:

  1. Do not scratch or rub the lesion.
  2. Wash the area with a mild cleanser or a no-soap cleanser daily for two months.
  3. Keep the area clean and dry.
  4. Maintain proper genital hygiene.
  5. Practice safe sex and use barriers.
  6. Always clean and wash genitals after sexual encounters.
  7. Reduce exposure to irritants.

Get urine routine and microscopy and urine culture tests done.

Also, get VDRL (venereal disease research laboratory test) or RPR (rapid plasma reagin) test and HSV 2 (herpes simplex virus type 2) IgM (immunoglobulin M) or IgG (immunoglobulin G) tests done.

I have reviewed the attached results (the attachment removed to protect the patient’s identity). Results are negative for both tests (HSV 2 and syphilis). HSV 2 (herpes simplex virus type 2) and syphilis have been ruled out. Follow up with urine culture reports.
