Hello doctor,
My son is five years old. He had a fever which went away automatically. We visited the doctor and he prescribed Mefenamic acid, Paracetamol, and Cefpodoxime after blood and urine tests. He also advised a urine culture. Now he does not have any fever. He said if it happens in the future then we can go for an ultrasound. Is there anything serious for which an ultrasound is required?

As per the report, there is a probability of urine infection as the total count of colonies is 50000 only so it is not a definite UTI. Also, it is not clear how the sample of urine was collected. Was it a bag collection or did you collect directly from the stream? Report after giving two doses of antibiotic has no significance as it is going to come negative. Apart from fever, did he complain of any urinary problems like burning, crying while passing urine, passing urine frequently, or passing less urine? As you have already started antibiotics, I suggest you give a complete course of antibiotics for seven days. Then wait for 48 hours after the last dose and repeat the culture. Regarding sonography, it is safer. There is nothing to worry about as the UTI is only a probability, not definite. Continue the antibiotic course and repeat the urine culture after 48 hours of the last dose. A clear catch sample is preferred (clean the penile region nicely before collecting the sample, let him pass a small amount of urine, and then collect the sample but do not include the last portion of urine).

Hello doctor,
The urine was collected in a cup supplied by the laboratory. Apart from the fever, he was complaining about a headache (only during the fever) and probably a sore throat and mouth thrush (after starting antibiotics). Now he has already completed 5 day doses of antibiotics and he is not having any such symptoms. Is there any chance that he suspected anything serious?

USG (ultrasonography) is to rule out any abnormality in the tube or it may be an infection only in the urinary bag. So any structural abnormalities that may give rise to repeated infection need to be ruled out as well as if there is any backflow of urine upward. These all are not serious but need to be ruled out and accordingly need to be managed. Some conditions may require a prophylactic treatment or may need more investigations. Again all these can be taken care of nicely so there is nothing to worry about but take all the actions as mentioned. Connect with me after you get a repeat urine culture report and USG KUB.

I was confused with MCUG (micturating cystogram) so I asked. Is MCUG required in this case? Or only USG KUB?

USG is non-invasive, go with it first. If something is positive then MCUG. Or if UTI recur even after USG is normal then go for MCUG. MCUG is an invasive procedure that needs a catheter to inject a dye into the urinary bladder and then to take X-rays. So first go for USG and repeat urine culture as I mentioned before.
