Can pregnancy occur in someone with a bulky uterus and fibroids?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I recently underwent a test due to experiencing very heavy menstrual flow, and I wanted to understand the cause. I am 32 years old, married, and ready to start having children.

The summary of my results indicates that my uterus is significantly bulky and contains multiple hypoechoic masses in the fundal (2.03x2.14 inches), posterior (2.87x2.59 inches), and anterior (1.69x1.50 inches) walls, distorting the endometrial echo stripe. No pelvic collection was observed. What steps should I take to improve my chances of getting pregnant and having children?

Kindly suggest.

Hello,

Welcome to icliniq.com.

I understand your concern.

Based on your ultrasound results (attachment removed to protect the patient's identity), it appears that you have uterine fibroids (multiple hypoechoic masses in the uterus). Fibroids are non-cancerous growths that can sometimes impact fertility, depending on their size and location. Here are a few suggestions that can help you:

How can fibroids affect pregnancy and fertility?

Fibroids can affect fertility and pregnancy in the following ways:

  1. Distorted endometrial stripe: This suggests that the fibroids may be affecting the inner lining of your uterus, which is essential for implantation.

  2. Large fibroids: The sizes (2.87 inches, 2.14 inches, 1.69 inches) indicate they are significant and could interfere with conception or cause complications during pregnancy.

  3. Heavy menstrual bleeding: A common symptom of fibroids, which may also signal hormonal imbalances.

  4. Reduced sex drive: This could be linked to fibroids, hormonal issues, or stress related to fertility concerns.

The next steps you can follow to improve your chances of pregnancy are:

Consult a fertility specialist or gynecologist who can assess whether the fibroids need treatment before you try to conceive. They may recommend further tests, such as a hysteroscopy or MRI (magnetic resonance imaging), to determine how much the fibroids are affecting your uterus.

Treatment options include:

  1. Medications: Some hormonal treatments can shrink fibroids, but they may not be ideal if you want to conceive soon.

  2. Surgical options (if needed): These include myomectomy. A procedure to remove fibroids while preserving the uterus, making it the best option for women who want to have children, and uterine artery embolization (UAE), which is not typically recommended for women trying to conceive, as it may affect the blood supply to the uterus.

Tips to boost fertility for conceiving naturally:

  1. Eat a healthy diet. Reduce processed foods and consume iron-rich foods to compensate for heavy bleeding.

  2. Exercise and weight management can help regulate hormones.

  3. Manage stress because chronic stress can affect ovulation and sex drive.

  4. Keep track of ovulation. Since fibroids can disrupt cycles, using ovulation-tracking tests or applications can help.

Low sex drive can be due to hormonal imbalance. Checking for estrogen or testosterone imbalance can help you. Making a few lifestyle changes like prioritizing intimacy, reducing stress, and getting enough rest can be beneficial.

Your fibroids are large and may require treatment before conception. A fertility specialist or gynecologist will provide guidance on the best steps forward. If you do conceive, you may require closer monitoring during pregnancy due to the fibroids.

I hope you find this information helpful.

Thank you.
