Hi doctor,

I am 36 years old and still have two baby teeth without permanent teeth (13 and 28). They are not in good health, and I never had the bone around, making the adjacent teeth want to close the gap. It seems that I do not have enough bone for a dental implant.

Now, this upper tooth is getting worse; a piece broke, and it starts to feel very uncomfortable. I already did some orthodontics (Invisalign) for aesthetics, but they did not address this problem. My current dentist cannot give me a solution.

  1. What are my options?
  2. Am I going for some very invasive and lengthy treatment?

You can solve this problem with a simple FPD (fixed partial denture), a bridge system, or an RPD (removable partial denture). But that would be possible only once these teeth are removed. The implant as a solution depends on the CBCT (cone-beam computed tomography) report and your overall health profile, including habits.

If there is good bone and you are healthy with no habits like smoking, an implant can be an excellent, viable option. Both procedures are non-invasive and cheaper than implants. However, to identify your chief problem precisely and put forward a treatment plan, I need your photographs of the area, and if you have radiographs, you can also send them.

Yes, I got it (attachments removed to protect the patient's identity); space is small, and there is a need for the distalization of molars to create space. Otherwise, the teeth cannot be placed.

However, a bridge can be designed, but the cutting on adjacent teeth would be more, and it needs a very skilled cosmetic dentist.
