How can a fungal infection of my second toe be treated?

Hi doctor,

I developed a fungal infection in the second toe of both legs. I have shared the images of my infected toes, where a slightly dark color is present in the second big toe. I tried Terbinafine ointment, but it did not show any effects.

Please suggest some medicines and how I should take care of them.

I understand your concern.

I looked over the image (attachment removed to protect the patient's identity). On the right foot, the second toe, I can see a slight circular darkening of skin with very mild superficial skin peeling. I could not see such a lesion on the left. Please mark an arrow or circle on the affected area on the image of the left foot.

How many days have you applied Terbinafine cream? It is a very mild, fungal infection on the right second toe. Is there any itching present? Please reply to the questions mentioned above for better evaluation and assessment.

I have marked a circle around the affected region and shared the photos. I have applied Terbinafine ointment on my toes for one week. Now I do not have any itching.

I understand your concern.

I looked over the image (attachment removed to protect the patient's identity). It could be a fungal infection, which is very mild and does not require any oral medication. Local cream application and a few preventive measures would be enough to treat the condition. You can do a KOH (Potassium hydroxide) smear test to confirm fungal infection and look for the fungal elements in the nail scraping sample.

You have used the correct amount of Terbinafine 1 % cream locally, but the duration of application is insufficient. Terbinafine 1 % is the best cream for local application in tinea pedis (a fungal skin infection that usually begins between the toes). I would suggest you follow a few things. Apply Terbiderm 1% cream twice a day for a total of 2 to 4 weeks, and make sure that the cream remains in contact with the skin for the maximum duration after application.

To minimize recurrence. Dry your feet and toes meticulously after bathing. If you wear shoes, avoid wearing occlusive footwear for long periods and thoroughly dry shoes. Avoid getting your toes wet and thoroughly dry them after coming into contact with water.
