Hi doctor,

It has been a week since I started getting this rash between my leg and groin, but only on one side. At first, it felt like my skin got torn there, and now it seems like it is spreading and is quite visible. I have not changed my diet. I wear new underpants daily and bathe daily. This is the first time it has happened, and moisturizing cream did not help. It is growing rapidly.

I have reviewed the attached image (attachment removed to protect the patient’s identity). Fungal infection, indeed, is very problematic if it becomes recurrent. First of all, keep these precautions in mind.

1. Do not keep the area wet at all. Use powder frequently to keep it dry.

2. Avoid itching, and refrain from scratching it.

3. Change the undergarments.

4. Do not apply general or steroid creams like Betnovate.

5. Do not stop the medicine once you are cured. Continue medicine for a long time to completely kill the fungus.

6. Avoid excessive sweating and wet clothes.

7. Check diabetes and thyroid issues. It can be recurrent even after proper treatment.

Now, onto the treatment part:

1. Ketoconazole cream and tablet Terbinafine 250 milligrams for two months.

2. Terbinafine cream and Fluconazole 150 milligrams twice a week for two months.

3. Tablet Itraconazole 200 milligrams once a day and cream for one month.
