Hello doctor,

My entire assisted living facility, where I work as a nurse, is currently facing a serious gastrointestinal outbreak. It started in the memory care unit yesterday and has quickly spread throughout our facility, which houses 87 residents. The symptoms include severe vomiting, intense diarrhea, and rapid dehydration. My primary concern is for our elderly residents who have multiple health issues. Despite our efforts to keep them hydrated with oral electrolyte solutions, many are unable to retain fluids and have become critically dehydrated. We've already had to send three residents to the hospital, but our local emergency room is becoming overwhelmed. With our medical director out of town, I am the charge nurse in charge of making containment decisions. What infection control measures should we put in place immediately? At what point should we consider sending additional residents to the hospital? How can we safeguard our limited staff from getting infected?
