Hello doctor,
I’m writing with concern about my husband, who is 29 years old. He has been experiencing persistent symptoms of gastroesophageal reflux disease (GERD), along with significant fatigue and unintentional weight loss. He often feels full after eating only small amounts and has started skipping meals to avoid discomfort.
I’m increasingly worried that this could be a sign of something more serious, possibly Barrett’s esophagus or even an early-stage malignancy.
Given his symptoms, do you believe he should have an urgent upper endoscopy, or are there other diagnostic tests you would recommend to help determine the cause?

Odynophagia is defined as sharp, substernal pain during swallowing that may limit oral intake, can be a sign of significant esophageal irritation or inflammation. While it is often seen in cases of severe erosive esophagitis, it can also occur in infectious esophagitis caused by organisms like Candida, herpes simplex virus (HSV), or cytomegalovirus (CMV), particularly in immunocompromised individuals.
Given the presence of persistent GERD symptoms, early satiety, unintentional weight loss, and fatigue, it would be appropriate to proceed with an upper endoscopy. This is the gold standard for evaluating ongoing reflux, difficulty swallowing, or unexplained weight loss. Endoscopy allows direct visualization of the esophageal and gastric lining, the ability to take biopsies (including for Barrett’s esophagus, eosinophilic esophagitis, or infection), and the ability to assess for any strictures, ulcers, or masses.
In some cases, other supportive tests may be recommended, such as

Esophageal pH monitoring (to measure acid exposure).

Barium swallow/videoesophagography (to assess motility and structural issues).
Given his symptoms, I would recommend discussing an urgent referral for upper endoscopy with his healthcare provider.
