Hello doctor,

I have been experiencing severe gastrointestinal issues for over a year now. Initially, the symptoms appeared only when I smoked, but they persisted daily even after I quit smoking.

I suffer from severe constipation and can only relieve myself by consuming nicotine and caffeine immediately upon waking. Additionally, I experience intense migraines, abdominal pain, and such significant bladder weakness that I frequently fear I might have an accident.

Despite numerous visits to doctors and various tests, all my blood work and examinations have returned negative results. Do you have any suggestions on what this could be?

I am very sorry to hear about your medical condition. Do not worry, I am here to support you. Based on your symptoms, it appears you may have developed peptic ulcer disease, potentially as a side effect of smoking, which can cause arteriosclerosis (Arteriosclerosis is the thickening and hardening of artery walls, which can restrict blood flow and lead to cardiovascular problems) affecting your brain and other systems. Please continue with your current medications and consider adding the following:

  1. 20 ml of syrup Gaviscon taken orally three times a day.

  2. Two tablets of Clarithromycin (antibiotic) 500 mg taken twice a day orally.

  3. 20 ml of syrup liquid Paraffin taken twice a day orally.

  4. One tablet of Vasoprin (Isosorbide mononitrate 60 mg and Aspirin 75 mg) 75 mg taken orally once a day, all for two weeks. Consult your doctor and take the medicines accordingly.
