Hello doctor,
I am a 35-year-old female. My psoriasis has exploded since I started taking thyroxine for Graves’ disease! I have developed huge plaques that bleed through my clothes. My endocrinologist says the dose cannot be lowered because my TSH is still high (28), but my skin feels like it is on fire. The Thyroid eye disease is bad enough, but now I also have psoriasis patches on my eyelids. Even makeup cannot cover the patches on my face, and I had to quit my teaching job because of it.

Psoriasis flaring up after starting Thyroxine for Graves' disease is a tough situation, especially with TSH(thyroid-stimulating hormone) still at 28. Thyroid hormone fluctuations, autoimmune activity, and stress can all worsen psoriasis. Since stopping or lowering Thyroxine is not an option right now, the focus should be on managing the psoriasis aggressively while your thyroid levels stabilize.
Seeing a dermatologist as soon as possible is important to start stronger treatments like topical steroids, vitamin D analogs (calcipotriol), or even systemic options if the plaques are severe. Moisturizing constantly with thick, fragrance-free creams can help reduce cracking and bleeding. Checking Vitamin D levels and considering UVB (ultraviolet)light therapy could also be beneficial. An anti-inflammatory diet that reduces gluten, dairy, and processed foods might help calm the immune response.
Psoriasis on the eyelids needs extra care since strong steroids cannot be used there. Tacrolimus ointment is often a safer option. If Graves’ eye disease is also worsening, an ophthalmologist should be involved to prevent complications.
Thyroid medication might be adjusted once levels stabilize, but switching to a different formulation like Tirosint could be considered if sensitivity to additives is contributing to inflammation. Since stress worsens both psoriasis and Graves’, trying to find stress relief through meditation, gentle movement, or even therapy could be helpful.
This is a frustrating and exhausting combination of conditions, but there are treatment options to manage psoriasis while staying on track with thyroid treatment. Please let me know how things progress and if any specific symptoms are worsening.
