My one-month-old, who is exclusively breastfed, has intermittent green poop. The pediatrician thinks it might be due to CMPA, but our baby has no other symptoms. What is the likelihood that the green poop is from CMPA rather than from bile?

I understand your concern about green stools in a one-month-old. It can be unsettling, especially when CMPA (cow’s milk protein allergy) is mentioned. Let us break it down:

Bile pigments give stool its color. When digestion is fast or variable, stool can appear green instead of yellow or brown. This can happen due to immature gut function, changes in feeding patterns, or a foremilk–hindmilk imbalance (more foremilk, which is higher in lactose and passes more quickly through the gut).

In an otherwise healthy, thriving, breastfed infant, green poop alone is usually normal and not a sign of disease. CMPA in breastfed babies is less common (about 0.5 to one percent of exclusively breastfed infants) and usually does not present with green stools alone. Typical symptoms include:

  • Blood or mucus in stools.

  • Diarrhea or very frequent watery stools.

  • Eczema or skin rashes.

  • Reflux, irritability, excessive crying, or colic.

  • Poor weight gain or feeding difficulties.

If your baby’s only “symptom” is intermittent green stool without blood, mucus, distress, skin issues, or growth problems, then CMPA is much less likely.

Green stools alone in a thriving one-month-old are most often normal and related to bile or feeding patterns. CMPA is possible but unlikely in the absence of other symptoms.

If symptoms such as:

  • Blood in stools.

  • Persistent diarrhea.

  • Poor weight gain, or

  • Significant fussiness appears, then elimination of dairy from your diet could be considered under medical supervision.

For now, I would advise monitoring rather than assuming CMPA.
