I have heavy flow with clots. Is it an early miscarriage?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello, doctor.

My period is 25 days late. I got six negative pregnancy tests taken on day eight, day 14, and day 21 after missing my period.

Now I am experiencing extremely heavy bleeding with clots soaking through multiple pads, severe cramping, feeling lightheaded, and nauseous.

Could I be experiencing a miscarriage that was too early to detect?

Kindly suggest.

Hello,

Welcome to icliniq.com.

I understand your concern.

I can feel how frightening and draining this must be for you; waiting through a late period, facing repeated negative pregnancy tests, and now suddenly experiencing such heavy bleeding and severe cramping.

What you are going through is not easy, and it is absolutely valid to feel worried that this might be more than just a late period. You are right to wonder about an early miscarriage.

Sometimes, very early pregnancies, called “biochemical pregnancies,” can end before the hormone levels rise high enough to be picked up on a home pregnancy test. In such cases, what follows may look like a very heavy, painful period with clots.

However, other causes can also present this way:

  1. A delayed but heavy period.

  2. Hormonal imbalance.

  3. Thyroid issues.

  4. Uterine conditions (such as fibroids or polyps).

Right now, what matters most is your safety.

Soaking through multiple pads in a short period, feeling lightheaded, and experiencing severe cramps are red flags for significant blood loss. You need urgent medical attention. Please go to the emergency room as soon as possible.

A doctor there can check your hemoglobin, do an ultrasound to look at your uterus, and confirm whether this is a miscarriage or another cause.

They can also give you medication to help slow or stop the bleeding, manage your pain, and provide IV (intravenous) fluids if you are losing too much blood.

Going forward, once you are stable, your doctor may suggest follow-up tests (hormone levels, thyroid function, ultrasound) to understand why your cycle was delayed and help prevent this from happening again. For now, the best step is to seek emergency care right away.

I hope this helps.

Thank you.
