Hello doctor,

I have been suffering from frequent belching and loss of appetite for the past twenty days. The main symptoms are repeated burping and reduced appetite. I am currently taking Esomeprazole with Domperidone before breakfast and dinner, and a digestive enzyme syrup twice daily, but there has been no improvement.

Kindly advise.

Frequent belching and loss of appetite for about twenty days are most commonly related to acid reflux, gastritis, functional dyspepsia, excessive gas formation, or infection with Helicobacter pylori (H. pylori), especially when symptoms do not improve with medications such as Esomeprazole with Domperidone and digestive enzyme preparations. Anxiety, irregular meal timings, eating too quickly, consumption of carbonated drinks, smoking, alcohol intake, or excessive tea and coffee consumption can also worsen burping and reduce appetite.

As there has been no significant improvement despite treatment, further evaluation is advisable. Recommended investigations may include Helicobacter pylori testing and basic blood tests, such as a complete blood count (CBC), to rule out infection or other underlying causes.

In the meantime, it is advisable to eat small, frequent meals; avoid spicy and oily foods; limit carbonated beverages; and avoid lying down immediately after meals. Chewing food slowly and thoroughly can also help. Meals should not be skipped, even if appetite is reduced.
