Hello doctor,

I have had a toilet tissue problem for a couple of days, giving me so much pain after completing bowel and it feels like swelling is also present.

Right now, I am using Preparation H hemorrhoid ointment and capsules in the jack-to-tool area.

Kindly help.

Hemorrhoids can be distressing, as they cause pain that can sometimes affect your quality of life. You may have an external hemorrhoid. For the acute management of pain, apart from using hemorrhoid preparation ointment, you can also do a sitz bath. This would help reduce the venous congestion in the prolapsed mucosa (hemorrhoid), which causes the pain, relieve the pain, and aid in the reduction of the hemorrhoid.

However, for long-term management, you would need to increase the fiber in your diet (eat food or fruits with high fiber content), as this would help reduce the likelihood of you becoming constipated, which can lead to the hemorrhoid prolapsing out and causing pain. Also, you would need to increase your fluid or water intake to at least 3.5 liters per day and ensure you are active (at least seven to ten thousand steps) during the day.

This also helps to prevent constipation and, by extension, the hemorrhoid from becoming worse. If none of these lifestyle and dietary modifications are helping, then you would need to come to the ER (emergency room) for examination. Occasionally, we start you on flavonoid-containing medications like Daflon (Diosmin and Hesperidin), and occasionally, you may require a surgical correction.

For now, a sitz bath and application of the hemorrhoid cream should help. Keep me updated.
