Hello doctor,

My father is 58 and has liver cirrhosis. Over the past week, he has been very forgetful, speaking in irrelevant ways, and sometimes seems confused about where he is. Yesterday, he even developed mild hand tremors.

The local doctor mentioned overt hepatic encephalopathy and started Lactulose. Our concerns are:

  1. How can we tell whether this is worsening or just temporary confusion?

  2. Should we take him to the hospital immediately if he becomes drowsy?

  3. Can dehydration make this condition more serious?

Your father’s symptoms, forgetfulness, confusion, irrelevant speech, and hand tremors (called asterixis), are classic signs of overt hepatic encephalopathy (HE). This condition occurs when toxins such as ammonia affect brain function due to cirrhosis, and it is often reversible with proper treatment.

To determine whether he is improving or worsening, watch for specific changes in his behavior and alertness. Signs of improvement include clearer thinking, normal conversation, improved sleep patterns, and better coordination, while worsening signs include increasing confusion, excessive sleepiness, slow or absent responses, personality changes, or difficulty recognizing family members.

You should take him to the hospital immediately if he becomes difficult to wake or is unable to recognize family members. Seek urgent care as well if he cannot take medications safely or develops a fever, vomiting, or any bleeding.

Dehydration is a major trigger and can significantly worsen hepatic encephalopathy. It reduces the body’s ability to clear toxins, so ensuring adequate fluid intake, unless his doctor has advised fluid restriction, is very important.

At home, monitor for early warning signs such as sleep reversal, slurred speech, or slowed thinking. Also watch for worsening hand tremors and ensure that lactulose is producing two to three soft bowel movements daily, as this helps remove toxins from the body.
