Hi doctor,

I performed analingus on a sex partner two days ago. I am getting stomach cramps and diarrhea but no fever. Could it be hepatitis? I got one hepatitis jab for A and B. Is that enough to prevent hepatitis? Please help.

I have read the query, and I can understand your concern.

The plausible reason for current symptoms includes bacterial gastroenteritis due to ingestion of gut pathogenic bacteria including but not limited to E. coli (Escherichia coli), shigella, or intestinal parasites such as tapeworm, hookworm, Ascariasis, pinworm leading to diarrhea and stomach cramps. However, this does not exclude transmission of the infections commonly by fecal-oral transmission during rimming or analingus, such as Hepatitis A, Hepatitis E, Gonorrhea, Herpes virus, etc. At this time, you should get a stool test done for routine examination and culture and sensitivity and revert with reports. Hepatitis is unlikely to be symptomatic this early, but the risk of infection does exist. The primary symptom would be jaundice, besides gastrointestinal symptoms.

As for hepatitis A and B vaccination, a single dose cannot protect you from hepatitis A or B. CDC (Centre for Disease Control), WHO (World Health Organization), and other national guidelines recommend restarting the vaccination immediately. Kindly consult your neighboring health center to restart the vaccination, as you risk contracting these infections. Kindly visit at the earliest. Twinrix (the combination Hepatitis A and B) vaccination should be administered (20microg/ml) 1 ml, preferably intramuscularly, either as three doses as the initial dose, one month, six months, or four doses as the initial dose, seven days, 21 to 30 days, and 12. Please revert at the earliest after giving samples for testing and with the laboratory test reports. Consult a specialist doctor, talk to them, and then take the medication.

The patient should start the following medicines immediately after giving blood and stool samples- 1. Capsule Saccharomyces Boulardii 250mg, one capsule twice daily for five days. 2. Capsule Racecadotril 100mg, one capsule twice daily for three days. 3. Oral rehydration solution (ORS, WHO formula) three to four liters daily for three days. 4. Tablet Drotaverine 80mg as required but not more than thrice daily. Consult a specialist doctor, talk to them, and then take the medication.

Preventive measures

I suggest you avoid anal and oral modes of sexual intercourse and complete Hepatitis A and B vaccinations.
