Hello doctor,

I am 26 and have been experiencing daily hives that appear and disappear without any apparent trigger. My IgE levels were elevated last month. I have tried higher doses of antihistamines like Cetirizine and Fexofenadine, but I still experience frequent flare-ups. Occasionally, I also get lip swelling and mild chest tightness.

Could this be chronic spontaneous urticaria? If antihistamines are not effective, would Omalizumab be the next step in treatment? Should I consider further allergy testing, or could this be autoimmune-related?
