Hello doctor,
I am a 25-year-old man. About 11 years ago, I noticed that one of my testicles was larger than the other. It is quite a bit bigger, and I have noticed that its size can vary, sometimes it is harder, and other times it is softer. Also, just before that, I had a severe case of cellulitis in my hand and arm. I have been worried about this and know I should have checked it out long ago. Can you tell me what this might be?

A swelling in the testicular area or scrotum can be either a hydrocele (accumulation of fluid inside the sac) or an inguinal hernia, which is caused by a portion of tissue or organ dropping down through a gap in your abdomen.
The best course of action would be to get an ultrasound and a physical examination to confirm the diagnosis. Treatment for both conditions is surgery. I suggest you do not delay it for too long. You can differentiate between the two conditions yourself by checking If it completely goes inside and becomes normal when you lie down, it is likely a hernia.
