What is the treatment for high calcium and low vitamin D levels?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My 12-year-old daughter had recent laboratory tests done after increasing depression and anxiety. She also developed a muscle tic. Anger and irritability that is worsening. Basic personality changes in a short amount of time. Her reports showed a high calcium level of 11, a high total protein of 8.2, and a lower Vitamin D of 31. She is having decreased appetite, she is irritable, and her anger levels are very high. Mood swings out of normal for her. She is crying about everything, even the littlest things, which seems over the top. Her doctor is telling me everything is normal, but everything I read says this is what could be leading to all these symptoms she is having. Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

I can understand your concern.

She has hypercalcemia which is likely a side effect of either Focalin or Guanfacine or a combination of both or as a result of previous medications. This is a tricky situation since if you treat it with vitamin D, it will further increase her calcium.

My suggestion would be to admit her to the hospital and get the hypercalcemia, treated with IV (intravenous) fluids.

Further investigation will be done after that.

I would suggest getting an opinion from a pediatrician would be better.

  • Do not try to treat her vitamin D deficiency.
  • Drink water and do not consume juice.

I hope this information will help you.

Thank you.

Patient's Query

Hello doctor,

Thank you for the reply.

Her pediatrician is the one who says it is not high and that anything under 14 is fine for her age. This is why I am questioning it because her symptoms are not fine, we know at home it is not her normal personality. Do I take her to a different pediatrician? Kindly suggest.

Thank you.

Hello,

Welcome back to icliniq.com.

I think the pediatrician should be changed to another one who understands the significance of raised calcium, and the associated symptoms from it and also discuss the issue with a psychiatrist since they should understand the side effects of their medications and sort it out.

I hope this information will help you.

Thank you.
