Hi doctor,
My son is one month old and has been diagnosed with coronal hypospadias. Testis is well located. The urine stream is straight. The foreskin is covered only 3/4th of the front part of the penis. Foreskin is not grown at the lower part of the penis. Penile length is around 0.78 inches. Is it micropenis? Can surgery correct this? In which month can we get it operated? How severe is it? And does it cause any problems in his puberty? I have attached photos of his parts for your reference. I am very much worried about this problem and his penile length.

1. I have reviewed the attached image (attachment removed to protect the patient’s identity). As per the length and photos you sent, it is not micropenis.
2. Also, do not worry about the hypospadias. As of now, surgical correction is not required. However, depending on the clinical scenario, a minor reconstruction surgery may be necessary once the child grows to about eight months or one year. It would help if you did not do circumcision for this child, as that part of the skin may be required for reconstruction.
3. Hypospadias in itself does not cause any problems with puberty. Still, if the hypospadias is very severe, there may be problems in the pattern of ejaculation, which in your case is currently difficult to say as the child is one month old.
