Hello doctor,

I have been previously diagnosed with secondary hypothyroidism with low TSH (thyroid stimulating hormone), T4, and T3 levels. I am taking thyroxine for this. And both T4 and T3 are now at the level above the reference ranges.

I have symptoms of exhaustion or tiredness, very significant short-term memory loss, right flank pain, stiffness in my whole body which lasts all day and feels like my bones are sore rather than joints, knee pain (clicking), pain around the area of the spleen although the doctor does not think it is enlarged, headaches (particularly on movement), dizziness, fainting, palpitations, high blood pressure, breathlessness, constant nausea, abdominal pain, gas, constipation, poor ability to exercise.

I gave birth four to five months ago to my second child. It was a normal birth with normal blood loss.

1. Sodium 141 mEq/L (normal range - 133 to 146).

2. Potassium 4.9 mEq/L (normal range - 3.5 to 5.30).

3. Creatinine 73 mg/dL (normal range - 44 to 80).

4. GFR 79 mL/min (advised technically normal but low for me as previously it was over 90 mL/min).

5. Hemoglobin estimation g/dL (normal range - 120 to 150).

6. WBC is high 10.05/mm cells (normal range - 4 to 10).

7. Platelet count is high 430 mcL (normal range -150 to 410).

8. Hematocrit 0.402 L/L (normal range - 0.36 to 0.46).

9. RBC high 5.14 million/mm cells (normal range - 3.8 to 4.8).

10. MCV high 78 fL (normal range - 83 to 101).

11. MCH low 25.3 fL (normal range - 27 to 32).

12. MCHC 323 fL (normal range - 315 to 345).

13. RDW 14.3 fL(normal range - 11 to 16).

14. Neutrophil 6.13 mL (normal range - 2.0 to 7.0).

15. Lymphocyte is high 3.1 mL (normal range -1.0 to 3.0).

Your mentioned CBC (complete blood count) report is not much altered. Total WBC (white blood count) and platelet count are borderline and cannot be considered high. Your RBC count is a little high and your MCV is low, so you can check minor hemoglobinopathy by hemoglobin electrophoresis by HPLC (high-performance liquid chromatography) method. As you have tiredness and joint pain complaints investigate serum calcium estimation, RA (rheumatoid) factor, ANA (antinuclear antibody) estimation, SGPT (serum glutamate pyruvate transaminase), and SGOT (serum glutamic oxaloacetic transaminase).

For weakness, if needed EMG (electromyography) and NCV-like studies can be done to check the muscular problem. For high blood pressure, antihypertensive medication can be started. Investigate with EKG (electrocardiography) as well to check the cardiac cause. Avoid stress if you are taking it.
