Hi doctor,

I am a 41 years old female, and I am suffering from chronic ITP (idiopathic thrombocytopenic purpura) for 13 years. I removed the spleen 10 years ago.

I have a history of bleeding, high pulse rate, loss of consciousness, spots on the skin, and inflammation of muscles and joints that caused an increase in temperature (104 F). So they gave me Cortisone and it raised my blood glucose and blood pressure.

I am currently under Cortisone, Neural, Azathioprine, CellCept 500 mg, Solupred 20 mg, Versapenia 50 mg, Eposino (epoetin alpha), and Epiao 4000 iu. But my blood platelets are not exceeding 10,000 and my hemoglobin now is 7, which is my current problem.

First, chronic ITP doctors were not able to detect my blood type so they used to do blood transfusion of type O in order to increase the hemoglobin. But now they are not able to do so, because of the high antibody count in my blood even after intake of cortisone and antibiotics. So they cannot correct my hemoglobin at all.

Your problem is complex.

I suggest you start the medication as follows. Consult your physician and take the medications only with their consent.

  1. Tablet. Prednisolone 20 mg once daily.
  2. Tablet. Mycophenolate mofetil 1000 mg once daily.
  3. Capsule. Cyclosporine 100 mg once daily.
  4. Tablet. Azathioprine 100 mg once daily.
  5. Injection. Vitamin B12 600 mg once daily for 15 days.
  6. Injection. Vitamin B6 pyridoxine 50 mg once daily for 15 days.
  7. Tablet. Folic acid 5 mg once daily.
  8. Tablet. Vitamin C any preparation once daily for 15 days.
  9. Vitamin D 5 mg once daily permanently.

After 15 days, do the following laboratory investigations.

  1. CBC (complete blood count).
  2. Reticulocytes.
  3. ESR (erythrocyte sedimentation rate)
  4. LDH (lactate dehydrogenase)
  5. Creatinine.
  6. Cholesterol.
  7. Atypical antibody detection.
  8. Direct antiglobulin test.
  9. Urine culture.

Get back to me with the reports after 15 days.

Hi doctor, thank you so much for the fast reply.

Unfortunately, I am already taking these medications for four months, but there is no improvement in my case or in the test results (as I already do the needed tests every month). So I would like to know if there is any new treatment or any other thing to do.

I can understand your concern.

I suggest the use of Rituximab injection. Atypical antibodies should be identified to precisely match blood and avoid minor blood group mismatch.

The medicine I prescribed should be taken as I advised and the routes of administration as I advised.
