Is it safe to start fertility medication without tests?

I have been trying for the last year to conceive a baby, but no result. I would like to take medication to boost my fertility. Need a doctor's prescription for the fertility medication. There are no other issues regarding the period cycle or health.

Fertility medications cannot be prescribed without a thorough evaluation. This involves identifying the exact cause of difficulty in conceiving, whether it is due to female or male factors. Among female factors, ovulatory dysfunction is the most common cause.

Tubal factors, such as tubal blockage, can also cause difficulty in conceiving and therefore require evaluation. I understand that your previous menstrual cycles were regular, but this time you experienced a delayed cycle, which you have noticed for the first time.

Delayed cycles may indicate ovulatory dysfunction, which can occur due to hormonal disturbances such as thyroid or prolactin imbalance, or due to conditions like PCOS (polycystic ovary syndrome). I also note that you are overweight for your height, which can contribute to a hormonal imbalance.

For you, the doctor may suggest several tests to better understand the condition. These may include blood tests such as hemogram, lipid profile, liver function test (LFT), and kidney function test (RFT) to assess your overall health. A 75 g oral glucose tolerance test (fasting, one hour, and two hours) and HbA1c (glycated hemoglobin) may be done to evaluate blood sugar levels.

Your doctor may also check serum calcium, vitamin D, and Vitamin B12 levels to rule out deficiencies. Fasting and post-meal insulin levels may be tested to evaluate insulin regulation. Hormone tests such as FSH, LH, free testosterone, estradiol, prolactin, and thyroid profile (T3, T4, TSH, Anti-TPO) are usually done on the second or third day of your next menstrual cycle.

An ultrasound of the abdomen and pelvis may be recommended to examine the uterus and ovaries. Your husband may also need a semen analysis to evaluate sperm health. If required, further tests like endometrial sampling and HSG (hysterosalpingography) may be done to assess the uterine lining and check whether the fallopian tubes are open. These tests help identify any hormonal, metabolic, or fertility-related issues and guide appropriate treatment.

Kindly take a tablet of folic acid 5 mg once daily until you conceive.

The following lifestyle measures may help improve your condition:

  1. Try to reduce excess weight, as it can help balance hormones.

  2. Maintain a menstrual cycle diary to track your periods and identify irregularities.

  3. Engage in regular exercise for at least one hour daily, such as walking, jogging, or yoga.

  4. Follow healthy dietary habits by eating balanced meals, limiting processed foods, and including more fruits, vegetables, and whole grains in your diet.

These measures can help support hormonal balance and reproductive health.

I would also like to know a few more details:

  1. Do you have acne or unwanted facial hair growth?

  2. Was there any recent weight gain?

  3. What is the average coital frequency per week?

  4. Do you have vaginal discharge or frequent lower abdominal pain?

  5. Have you had tuberculosis or any other chronic medical condition in the past?

  6. Has your husband already undergone semen analysis?

  7. What is his age and occupation?

Since it is the third day of your period today, you can get the above-mentioned tests done and consult a fertility specialist with the reports for further evaluation and management. Treatment will depend on the underlying cause. In about 10–20 % of cases, no specific cause is identified, which is known as unexplained infertility.

In such situations, fertility specialists may suggest assisted reproductive techniques to improve the chances of pregnancy.
