Can having the flu at 8 months pregnant harm my baby?

Hello doctor,

I am currently eight months pregnant and was diagnosed with Influenza A four days ago after developing a sudden fever of 103°F, severe body aches, headache, and now a persistent, painful cough that puts a lot of pressure on my abdomen.

I went to urgent care, where I tested positive and was prescribed Oseltamivir (75 mg twice daily), though they mentioned it would have been more effective if started earlier. My fever has decreased to around 100°F with Tylenol, but I still feel very unwell, extremely fatigued, short of breath even with minimal activity, and my oxygen saturation is around 94 percent, which I was told is borderline.

My OB checked the baby’s heartbeat yesterday, and it was normal, but I am still very concerned about the potential effects of the flu on my baby, especially this far along in pregnancy. I have been doing my best to stay hydrated and rest, but nausea is making it hard to drink enough fluids. I also read that the flu during pregnancy can increase the risk of complications like preterm labor or birth defects, which has me very anxious.

I am currently managing symptoms with Tylenol, fluids, and rest, but on day four, I am still feeling quite ill. At what point should I go to the hospital if symptoms do not improve? And is there a risk of harm to the baby from the flu at this stage in pregnancy? Please help.

To provide some clarity: Influenza (flu) is a contagious respiratory illness during pregnancy that can sometimes increase the risk of complications such as preterm labor, low birth weight, and, in rare cases, birth defects, particularly if the fever is prolonged and not well-controlled.

It is important to seek immediate medical attention if you experience any of the following:

  • Difficulty breathing or shortness of breath.

  • Chest pain or severe, persistent coughing.

  • Decreased or no fetal movement.

  • A high fever that does not respond to Acetaminophen (Tylenol).

  • Signs of dehydration, such as dizziness or significantly reduced urine output.

Acetaminophen is generally safe during pregnancy and effective for managing fever and body aches. Continue using it as directed. Be sure to avoid NSAIDs (Non-steroidal anti-inflammatory drugs) like Ibuprofen. The antiviral medication you were prescribed (Oseltamivir or Tamiflu) is also considered safe in pregnancy and can help reduce the severity and duration of the flu.

It is essential to rest as much as possible, stay hydrated, and monitor your symptoms closely. If you begin to feel worse, show any signs of preterm labor, or notice a decrease in fetal movement, please do not wait; contact your OB (maternal-fetal medicine specialist) or go directly to the emergency room. Early intervention and monitoring can make a big difference for both you and your baby’s well-being.
