I have a Mirena intrauterine device (IUD), and two days after having rough sexual intercourse for three consecutive days, I noticed some bleeding in the vagina.

One common cause is cervical irritation or minor trauma. Rough intercourse can cause small tears or irritation to the cervix or vaginal walls, especially when an IUD is in place. This type of bleeding is usually mild and not a cause for concern.

Another possibility is hormonal spotting related to the Mirena IUD. Even after months or years of use, occasional irregular spotting can still occur. The Mirena IUD releases the hormone Levonorgestrel, which causes the uterine lining to remain thin. As a result, the lining can be more sensitive and prone to light bleeding from friction.

In rarer cases, bleeding could be due to a shift in the position of the IUD.
