Can HRT helps improve energy and mood?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My father has been diagnosed with early-stage Parkinson’s disease and recently began treatment with carbidopa-levodopa (a dopaminergic agent). My mother, who is his primary caregiver, is struggling with exhaustion and appears to be depressed. She is 63 years old, postmenopausal, and has arthritis.

  1. Is caregiver fatigue common in wives of patients with Parkinson’s disease?
  2. Could hormone replacement therapy or antidepressants safely help improve her energy levels and mood?

Kindly help.

Hello,

Welcome to icliniq.com.

I have read your query and can understand your concern.

Thank you for reaching out. Your concern for both of your parents is truly commendable.

Yes, caregiver fatigue is extremely common, especially among spouses who care for a loved one with a chronic neurodegenerative condition such as parkinson’s disease. Your mother’s experience of exhaustion, low mood, and physical strain is not unusual, particularly given her health challenges, including arthritis and postmenopausal symptoms.

Here is what is important to understand:

  1. Caregiver burnout is real. Emotional, physical, and psychological fatigue often develop gradually. Many caregivers report symptoms of depression, anxiety, and worsening of preexisting health conditions due to prolonged stress and limited time for self-care.

  2. Hormonal changes after menopause can contribute to low energy, poor sleep, and mood disturbances. If her menopausal symptoms are still significantly affecting her quality of life, hormone replacement therapy (HRT) may be considered. However, it should be carefully evaluated in consultation with her physician, taking into account her age and medical history, especially regarding arthritis.

  3. Antidepressants (like selective serotonin reuptake inhibitors—SSRIs) or mood stabilizers can be beneficial if there are signs of clinical depression. Many of these medications are well-tolerated and safe for individuals in her age group, even when used alongside medications prescribed for arthritis.

  4. Support systems and respite care are essential. Encourage her to explore caregiver support groups, professional counseling, or adult day programs that can offer temporary relief. Her health and emotional well-being are vital, not only for herself but also to provide continued care for your father.

I hope this helps.

Thank you.
