Hello doctor,

I had my period after a two-month gap, and it lasted for a few days. However, it started again and has not stopped since. The blood is black-tinged, and I am experiencing a lot of pain. Could you please help me understand what is going on?

A menstrual cycle longer than 38 days is considered infrequent or irregular. A normal cycle typically falls between 24 and 38 days, and the normal duration of menstrual bleeding is up to eight days. If bleeding lasts longer than eight days, it is considered abnormal.

Based on your complaint, you may be experiencing irregular and heavy menstrual bleeding. The causes for this could be varied and include:

  1. Endometrial polyps (growths in the inner lining of the uterus).
  2. Fibroids in the uterus (non-cancerous tumors in the uterus, especially submucosal or intramural types).
  3. Cervical polyps or fibroids (growths on the cervix or in the cervical area).
  4. Pelvic inflammatory disease (PID) (infection of the reproductive organs, often caused by sexually transmitted infections).
  5. Chronic cervicitis (long-term inflammation of the cervix).
  6. Thyroid disorders (conditions affecting thyroid function, such as hyperthyroidism or hypothyroidism).
  7. Prolactin disorders (issues with elevated levels of prolactin hormone, affecting menstrual cycles and fertility).

To help determine the cause, investigations may include:

  1. Serum prolactin levels.
  2. Serum FSH (follicle-stimulating hormone) and LH (luteinizing hormone) on the second day of your cycle.
  3. Pelvic ultrasound.

Management:

Continue taking the current medications prescribed by your doctor.

You may be advised to take the following:

  1. Tranexamic acid 500 mg, three times a day for five days, to help reduce heavy bleeding.
  2. Mefenamic acid 500 mg, twice a day for five days, to alleviate pain.
  3. Norethisterone 5 mg, three times a day for 21 days, followed by a seven-day pill-free interval. Afterward, repeat the cycle for three months, with 21 days of medication followed by seven days off.
