Can FODMAP sensitivity cause IBS-D symptoms?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 34-year-old female and have been struggling with IBS-D for years. I experience frequent loose stools, bloating, and urgency, especially after meals. Colonoscopy was normal. Stress makes it worse. I have tried probiotics and a gluten-free diet, but the relief is temporary.

  1. Are there any long-term medications that can control this?

  2. Could it be related to food intolerances like FODMAP sensitivity?

Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

I read your query and understood your concern.

I hear how exhausting it must feel to live with these unpredictable symptoms; it can take a real toll on daily life. From what you describe, your pattern fits irritable bowel syndrome with diarrhea (IBS-D), which is a chronic but manageable condition.

I suggest you undergo the following evaluations:

  1. Stool analysis, including osmotic gap to exclude malabsorption.

  2. Celiac screen including tissue transglutaminase immunoglobulin A (tTG-IgA) with total immunoglobulin A (IgA).

  3. Fecal calprotectin to rule out inflammatory bowel disease (IBD).

  4. Watch for red flags: blood in stool, nocturnal diarrhea, unintentional weight loss, persistent fever, or family history of IBD or colon cancer. These would require further evaluation.

Regarding food intolerances like lactose, high-FODMAP (fermentable oligosaccharides, disaccharides, monosaccharides, and polyols) foods:

  1. These are common triggers.

  2. The low-FODMAP diet should be applied as a short-term elimination phase (four to six weeks) with gradual reintroduction to identify personal triggers, not as a permanent strict diet.

  3. Keep a food and symptom diary to connect flares with meals.

I suggest these treatment options:

  1. Antispasmodics (e.g., Mebeverine, Dicyclomine) for cramps.

  2. Loperamide as needed for diarrhea control.

  3. Low-dose antidepressants like tricyclic antidepressants (TCA) (e.g., Amitriptyline) may help in IBS-D by calming gut sensitivity.

  4. Selective serotonin reuptake inhibitors (SSRI) (e.g., Sertraline) may help if constipation or anxiety dominates.

  5. Probiotics can be tried, but their effects are variable.

I also suggest stress management, such as mindfulness, yoga, cognitive behavioral therapy (CBT), and regular exercise. It is as important as food in IBS control.

IBS is chronic, but with the right combination of diet, medication, and stress management, symptoms can be significantly reduced. Please share any recent laboratory results with me when available. I will be glad to review and adjust the plan with you.

I hope that this answers your query.

Kindly follow up if you have more doubts.

Thank you.
