Hello doctor,
I am 41 and was recently diagnosed with Irritable bowel syndrome with diarrhea (IBS-D) after a colonoscopy ruled out other diseases. My main problem is a sudden urgency to pass stool, especially when I’m traveling or attending meetings.
I often experience abdominal gurgling sounds before an episode, which becomes very embarrassing in public situations. Sometimes, I also feel mild nausea and fatigue afterward.
My doctor prescribed antispasmodic tablets, but the relief has only been partial. I have noticed that my symptoms increase whenever I am under work pressure or do not get proper sleep.
Can stress alone worsen IBS-D symptoms this much? Are there long-term treatment strategies that focus on gut health rather than just temporary symptom control?

Your symptoms are typical of irritable bowel syndrome with diarrhea (IBS-D). In this condition, the intestine appears normal on tests, but the bowel becomes more sensitive and moves stool faster than usual, leading to urgency, abdominal gurgling, cramps, and loose stools.
Stress can indeed worsen IBS-D significantly. The gut and brain are closely connected through the gut–brain axis.
When you are under pressure, anxious, or sleep-deprived, the nervous system stimulates the intestine, increasing bowel activity and sensitivity. This explains why symptoms often appear during travel, meetings, or stressful work periods.
