Hello doctor,

I am having a very concerning lesion growing on the shaft of my penis. It has been there for around nine days. I went and got the works for STD tests, and everything came back negative. I attempted a home remedy and tried apple cider vinegar (ACV), as I was afraid it is genital warts and I was unsure about ACV coverage has as a cure-all. Needless to say, this helped at first. The lesion scabbed over and shrunk, and the swelling went away. The scab fell off a few days later and came back with a vengeance. The initial lesion doubled in size and is covered in a white layer that feels hard to the touch. The skin around the large lesion became rough, and smaller white bumps began to pop up and go away. I mean, literally, the smaller bumps will appear throughout the day and then vanish. I know this might not be a good enough description, but I am panicking over this.

Thank you for the reply.

I have sent you the picture. There is itching at times, and I have been experiencing pain in my left testicle for a couple of hours during the day. The pain is usually restricted to my left testicle, but on rare occasions, it occurs in my right testicle as well. The itching is something that has developed in the last two days. I had unsafe sex with the same person on two occasions three months ago. They claimed to be completely clean, and like many others, I foolishly engaged in unprotected sex. If the photo was not satisfactory or if you need more, please let me know. I really need help with finding a solution to make it stop worsening and to get it under control until I can see a doctor in person. Once again, thank you for your time. You have no idea how much it means to be able to talk to a doctor about this.

Presently, it appears to be in favor of irritant dermatitis, likely due to the application of vinegar. Therefore, you need to take the following medications and revert back to me in ten days:

  1. Take tablet Augmentin (Amoxicillin and Clavulanic acid) 625 mg thrice daily for seven days.
  2. Apply Mupirocin ointment twice daily for ten days.
  3. Please do not remove the scab and avoid exposing the lesion to water for ten days.
  4. Avoid any kind of sexual activity during the treatment.

I do not know about the status in your country or whether you can get it over the counter. The drug name for Augmentin is a combination of Amoxicillin and Clavulanic acid 625 mg. You can try finding these medications.

Thank you again for your time; I greatly appreciate it. I will look into those medications and keep you posted. I do have one last question. Do you think it looks like genital warts at all? I know you are favoring irritant dermatitis, which I am hoping is correct, but I understand that it is impossible to have a true diagnosis from a picture. I guess I am just hoping to calm my anxiety that I do not have human papillomavirus (HPV). You have been incredibly helpful. Have a great rest of your day.

At present, there are no morphological features suggesting it is a wart. Sometimes, vinegar-induced necrosis can effectively treat warts, but there is a higher risk of recurrence, secondary infection, and scarring.
