Hello doctor,

My query is that for the past few months, I have developed a condition in which, whenever I exercise or play football, my whole body starts itching very badly. It becomes almost unbearable. It also happens whenever I feel anxious, such as while driving a car or attending an interview.

I have taken Softin-F for the past three to four days, but it is not helping. Please suggest a solution.

Based on your history of severe itching triggered by exercise, football, anxiety, driving, and during the winter months, this pattern is very typical of cholinergic urticaria. It is a common condition seen in young individuals, especially males in your age group. This happens when your body temperature rises due to exercise, emotional stress, or sweating. It leads to the release of histamine in the skin. As a result, you may experience intense itching, a prickling or burning sensation, and sometimes very small red bumps that may be difficult to notice. Symptoms usually last 20 to 60 minutes and then settle on their own. Cold weather can reduce regular sweating, making sweat glands more sensitive when suddenly activated, which explains why it may worsen in winter.

Regarding your medication, Softin-F (a Fexofenadine combination) may not show significant improvement within just three to four days. Antihistamines usually need to be taken regularly for at least two to four weeks for proper control. Sometimes the dose needs adjustment or a switch to another non-sedating antihistamine under medical supervision.

You may take tablet Softin-F 180 mg in the morning and tablet Xylor (Levocitrizine) 10 mg at bedtime, as advised by your doctor. Continue this for one month and follow up. If there is no improvement, your doctor may consider other advanced treatment options.

For better control, start with a gradual warm-up before intense exercise instead of sudden heavy activity. Avoid very hot showers. Wear loose cotton clothing and stay well hydrated. Since anxiety also triggers your symptoms, practice slow, deep breathing before driving or attending interviews. Avoid overheating and very spicy foods if they worsen symptoms. Maintain proper nutrition and a healthy lifestyle, as overall well-being helps improve symptom control.
