What are the potential causes of persistent itching?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My sibling has been experiencing persistent, generalized pruritus for several weeks. The itching is most intense upon waking, resulting in skin excoriations, soreness, and post-inflammatory hyperpigmentation. She has undergone standard blood and urine tests, all of which were unremarkable. She has also received injections (details unknown) and has tried Rhizin and Wormplex without relief. Please tell me, what are the potential causes, and what steps can we take to find relief?

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have read your query and can understand your concern.

The following are the possible causes of persistent itching in your sibling's case

Dermatological conditions:

  1. Dry skin (xerosis): Often exacerbated by dehydration.

  2. Eczema (atopic dermatitis): A chronic inflammatory skin condition.

  3. Psoriasis: An autoimmune condition affecting skin cell turnover.

  4. Urticaria (hives): Typically an allergic reaction.

  5. Miliaria (heat rash): Common in hot, humid climates.

Allergic reactions:

  1. Food allergies, insect bites/stings, pollen, medications.

  2. Contact dermatitis: Reactions to cosmetics, fragrances, preservatives, etc.

Systemic diseases:

  1. Liver or kidney disease.

  2. Iron deficiency anemia.

  3. Diabetes mellitus.

  4. Thyroid disorders.

  5. Certain malignancies.

Neurological conditions:

  1. Multiple sclerosis.

  2. Nerve compression or damage (for example., pinched nerves, shingles).

Psychogenic factors:

  1. Anxiety, obsessive-compulsive disorder, depression.

General recommendations:

  1. Hydration: Maintain adequate fluid intake to combat dry skin.

  2. Moisturization: Apply fragrance-free, hypoallergenic moisturizers regularly.

  3. Stress management: Implement stress-reduction techniques (e.g., exercise, mindfulness).

  4. Symptomatic relief:

A. For insect bites: Topical hydrocortisone or oral antihistamines.

Important considerations that your sibling can take;

  1. Given the persistence of your sibling's itching despite previous treatments and normal blood/urine tests, a consultation with a dermatologist is strongly recommended.

  2. Further investigations may be needed to rule out underlying systemic or neurological causes.

  3. The treatments will vary vastly depending on the underlying cause.

  4. Do not self-medicate.

  5. Avoid scratching, as it can worsen skin damage and increase the risk of infection.

I hope this answers your query.

Please let me know if I can assist you further.

Thank you.
