Why am I having severe pain five days after IUD insertion?

Hi doctor,

I have severe endometriosis and got an IUD put in five days ago, on day three of my period. I am in severe pain, and I cannot walk without hunching. Paracetamol, anti-inflammatories, and Codeine are not effective. Is this normal, and will it pass, or do I need medical attention?

That kind of severe pain after an IUD (intrauterine device) insertion is not something that should be considered normal. Mild cramping for a few days is expected, but if you are having trouble walking, and even strong painkillers are not providing relief, then something more serious could be going on.

Possible causes include the following.

  1. A malpositioned IUD (intrauterine device).

  2. Uterine spasms from the insertion.

  3. The start of a pelvic infection.

  4. A uterine perforation.

With conditions like endometriosis, it can sometimes be difficult to separate the types of pain, but the severity you are describing goes beyond what is typical after insertion.

I suggest the following investigations for you.

  1. Pelvic ultrasound – To check the position of the IUD.

  2. Physical pelvic exam – To assess for tenderness, signs of infection, or malposition.

  3. CBC (complete blood count) – To check for signs of infection or inflammation.

  4. CRP (C-reactive protein) – To assess the level of inflammation in the body.

At this level of pain, it is not safe to wait it out at home. You need to be reviewed urgently, either by your gynecologist or at an emergency clinic. An ultrasound will help confirm whether the device is properly placed. If it is malpositioned, it may need to be removed or repositioned.

If an infection is present, antibiotics should be started right away. Pain relief with Paracetamol or Codeine alone is not enough in this situation.
