Patient's Query

Hi doctor,

I recently blood-tested for kidney function and my results are as follows

  1. Urea 6.0 mmol/l 1.7-8.4

  2. Creatinine 81 umol/l 62-115

  3. eGFR >60 ml/min >60

  4. Calcium 2.36 mmol/l 2.12-2.52

  5. Inorganic phosphate 1.35 mmol/l 0.78-1.65

  6. Uric acid 0.41 mmol/l 0.20-0.42

  7. Sodium 140 mmol/l 137-150

  8. Potassium 4.4 mmol/l 3.5-5.3

  9. Chloride 104 mmol/l 96-108

  10. Urine microalbumin 164.0 mg/l

  11. Urine creatinine 7.02 mmol/l

  12. Microalbumin: creatinine ratio 23.4 mg/mmol

As far as I have noticed, there is no frothing in my urine (other than on occasions when I do not hydrate enough and there are bubbles), no blood in the urine, and my urination frequency has not been abnormal nor have I felt pain anywhere when urinating. But I must say that lately in the past few weeks, I have felt lethargic and tired when I get less than eight hours of sleep.

I am currently not under any medications other than vitamins.

What do my results say? Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

I saw your reports and as per you, there are no issues related to urination. You have attached two reports (attachments removed to protect patient identity). There is variation in urine analysis in these two, one showing protein 1 plus and the other showing negative. Also the urine for microalbumin is different from the report you have mentioned in history. Could you clear this doubt, please?

Rest your reports are in the normal range except for having high LDL (low-density lipoproteins) and cholesterol. This needs a healthy lifestyle and then repeating the same lipid profile after three months.

Thank you.

Patient's Query

Hi doctor, thank you so much for the reply.

Please note that the one report is actually from last year, and the second report is the one for this year, and I attached them for comparison over the one-year period.

What bothers me are the following indicators,

  1. Elevated uric acid at 0.41 mmol/l (0.31 last year).

  2. Elevated microalbumin: creatinine ratio at 23.4 (0.5 last year).

  3. Protein 1+ (negative last year).

Also, my blood pressure is normal and I am not diabetic, although my cholesterol is 5.7 mmol/l.

Thank you.

Hello,

Welcome back to icliniq.com.

I checked your reports again. You have an increased urine albumin - creatinine ratio. This needs to be tested again after six weeks. If it is persistently high, I suggest you start with a tablet Telmisartan. We should follow up on this.

I suggest you follow a healthy lifestyle.

1. Low salt diet (less than 5 grams per day).2. Active lifestyle with 30 minutes walk daily.3. Keep weight under control.4. Avoid painkillers or any unnecessary over-the-counter drugs.5. Adequate fluid intake.

Repeat the urine albumin-creatinine ratio after six weeks and do one ultrasound of the kidneys.

Thank you.
